Farnsley Middle School

All students participating in athletics at Farnsley Middle School must have a completed JCPS/KHSAA sports physical on file before participating in any team activity—including tryouts, conditioning, or practice. The required JCPS 6-page physical packet must be filled out by a medical provider and includes: Sports Physical Exam Form Permission to Treat Form Consent to Participate JCPS Addendum and Insurance Verification Note: The 6th grade school physical does not meet the KHSAA athletic requirement.
